Hospital Reporting of Less Serious Adverse Events or Close Calls


(1)

In addition to Reportable Serious Adverse Events, Participating Hospitals are also encouraged to report less serious adverse events or close calls. Participating Hospitals should do so when they believe other organizations will benefit from the information.

(2)

To report such events, Hospital Participants should use the appropriate sections of the Event Report form. (3) Hospital Participants are not required by the Commission to provide written disclosure of less serious adverse events or close calls to patients or their personal representatives.
