Purpose

(1)

These rules establish a registry (the Oregon POLST Registry) within the Oregon Health Authority for the collection of POLST forms and the dissemination of information from POLSTs to help ensure that medical treatment preferences for individuals nearing the end of the individual’s life are honored.

(2)

The Registry may be operated and maintained by a contractor of the Authority.
