Reporting of Disciplinary Actions

The Board shall report to the Secretary of Health and Human Services in the manner prescribed by the Secretary the following information:

(1)

Any disciplinary action taken by the Board as the result of a formal proceeding concluded against a speech-language pathologist or audiologist.

(2)

Any dismissal or closure of the proceedings by reason of the speech-language pathologist or audiologist leaving the state.

(3)

Any other loss of the license of the speech-language pathologist or audiologist, whether by operation of law, voluntary surrender, or otherwise.
