Incidents

(1)

In the event of an incident, the transporter shall immediately notify all of the following:

(a)

Oregon Emergency Response System telephone: 1-800-452-0311 when initiating the call within Oregon; and 1-503-378-4124 when initiating the call from outside Oregon;

(b)

National Response Center (Telephone: 1-800-424-8802);

(c)

Waste Generator (Telephone: see manifest or other shipping papers).

(2)

The transporter shall note on the manifest the time and location of the incident and the type and amount of the hazardous waste which has spilled.

(3)

Within 30 days after the incident, the transporter shall file a Hazardous Materials Incident Report (DOT Form F5800.1) with the Oregon Department of Transportation.
