Placement on Supervisor Registry

(1)

To be placed on the Supervisor Registry, an active licensee must submit a request on forms provided by the Board.

(2)

Licensees on inactive status will be denied or removed from placement.

(3)

Licensees may request in writing to be removed from the registry.
