Approval of Treatment Providers


(1)

To be approved by the Board as a treatment provider, a provider must be:

(a)

Licensed as required by the jurisdiction in which the provider works;

(b)

Able to provide appropriate treatment considering licensee’s diagnosis, degree of impairment, and treatment options proposed by the independent third-party evaluator; and

(c)

Able to facilitate a urinalysis of the licensee at intake.

(2)

A treatment provider may not have a personal or professional relationship with a licensee.

(3)

The Board will maintain a list of treatment providers available to licensees upon request.
