(9) Surcharge to Support the Workforce Data Analysis Fund at Renewal — $4.
(10) CNA Certification for RN or LPN — $60.
(11) CNA Certification for Student Nurses — $106.
(12) Temporary Certificate for Active Armed Forces Military Spouse or Domestic Partner --- $50.
