Renewal Application

(1)

The biennial renewal application must be submitted to the Board with the appropriate fee and the pharmacist must attest that he/she has satisfactorily completed the continuing pharmacy education requirements.

(2)

The Board may randomly select and audit applications for renewal to verify completion of the CE programs reported on the application for renewal. Pharmacists whose applications for renewal are selected for audit must provide documentation of completion of the CE programs reported. A pharmacist who fails to provide the requested documentation to the Board or who fails to complete the biennial CE requirement may be disciplined for unprofessional conduct.
