A licensee must provide to the Authority a copy of the board certification, the Nuclear Regulatory Commission or Agreement State license, or the permit issued by a licensee of Broad Scope for each individual no later than 30 days after the date that the licensee permits the individual to work as an authorized user, or an authorized nuclear pharmacist pursuant to OAR 333-116-0040 (License Amendments)(2)(a) through (d).(2)
A licensee must notify the Authority by letter no later than 30 days after:(a)
An authorized user, an authorized nuclear pharmacist, a Radiation Safety Officer or an authorized medical physicist permanently discontinues performance of duties under the license or has a name change;(b)
The licensee’s mailing address changes;(c)
The licensee’s name changes, but the name does not constitute a transfer of control of the license as described in OAR 333-102-0305 (Special Requirement for a Specific License to Manufacture, Assemble, Repair or Distribute Commodities, Products or Devices Which Contain Radioactive Material: Specific Terms and Conditions of License); or(d)
The licensee has added to or changed the areas where radioactive material is used in accordance with OAR 333-116-0200 (Requirements for Possession of Sealed Sources and Brachytherapy Sources) and 333-116-0300 (Use of Unsealed Radioactive Material for Uptake, Dilution or Excretion Studies for Which a Written Directive Is Not Required).(3)
The licensee must mail the documents required in this division to the Authority for review.
