Provider Enrollment


(1)

Registered nurses and licensed practical nurses shall submit a copy of licensure every two years upon renewal by the Oregon State Board of Nursing to be enrolled or continue enrollment as a Division provider.

(2)

If the Division provider is a nursing employment/staffing agency, the agency shall:

(a)

Be licensed in the State of Oregon as an in-home care agency with a comprehensive licensure as defined in OAR 333-536 or Home Health Agency OAR 333-027;

(b)

Conduct a background check through the Department CRIMS;

(c)

Provide a copy of the state licensure to the Authority upon request.
