Eligibility for Case Management Services

To be eligible for case management services a person must:

(1)

Be 18 years of age or older;

(2)

Be eligible for OSIP-M; and

(3)

Meet the functional impairment level within the service priority levels currently served by the Department as outlined in OAR 411-015-0010 (Priority of Paid Services) and 411-015-0015 (Current Limitations).
