Renewal Of License Required


(1)

All physical therapist and physical therapist assistant licenses lapse on March 31 of each even numbered year, regardless of the initial issue date. Physical therapists and physical therapist assistants must renew their licenses to practice effective April 1 of each even numbered year. A license is considered lapsed if a completed renewal application is postmarked or electronically received after March 31 of an even number year. A person whose license has lapsed must immediately stop practicing as a physical therapist or a physical therapist assistant and shall not practice until the license is renewed.

The renewal period shall be January through March 31st of an even number year. The Board will attempt to provide renewal notification to all current licensees during the month of January.

If the completed license renewal application is postmarked or received by the Board after March 31st, in addition to the license renewal fee, the licensee is subject to a lapsed license renewal fee as provided in OAR 848-005-0020 (Board Fees and Refunds)(1)(e).

(4)

A licensed physical therapist must complete the renewal application process and pay the renewal fee provided in OAR 848-005-0020 (Board Fees and Refunds)(1)(c).

(5)

A licensed physical therapist assistant must complete the renewal application process and pay the renewal fee provided in OAR 848-005-0020 (Board Fees and Refunds)(1)(d).

(6)

Each currently licensed physical therapist and physical therapist assistant must complete the continuing competence requirements as provided in Division 35 of these rules before they may renew.
