Expiration and Renewal of License


(1)

Each license shall expire on the 31st day of December of each calendar year.

(2)

An agency shall submit a completed application for renewal on a form prescribed by the Division, accompanied by the required fee, to the Division not less than 30 days prior to the license expiration date.

(3)

The Division may issue a renewal license contingent upon evidence of the agency’s compliance with ORS Chapter 443 (Residential Care) and OAR chapter 333, division 027; attestation to the delivery of agency services to patient(s) during the last calendar year; and, if requested, receipt of an annual statistical report containing such information as may be prescribed by the Division.
