Proof of Coverage Terminations


When the insurer cancels coverage before the expiration of the policy term, the insurer must:

(1)

Provide written notice to the employer under ORS 656.427 (Termination of workers’ compensation insurance contract or surety bond liability by insurer)(1) and (3);

(2)

Retain a record of the written notice sent to the employer under ORS 656.427 (Termination of workers’ compensation insurance contract or surety bond liability by insurer), and proof of mailing of that notice, for inspection by the division; and

(3)

Provide notice to the director under ORS 656.427 (Termination of workers’ compensation insurance contract or surety bond liability by insurer) by EDI.
