Waiver of Minimum Service Obligation

The Commission may waive all or part of the fees and penalties owed to the Commission under the following circumstances:

(1)

Upon receiving written documentation acceptable to the Commission of the death of the participant; or

(2)

Upon receiving written documentation acceptable to the Commission of the total and permanent disability of the participant; or

(3)

After consideration by the Commission and the Oregon Board of Nursing of a written appeal from the participant for a waiver of the minimum service obligation.
