Supported employment services


(1)

“Supported employment services” means ongoing support services, including customized employment, needed to support and maintain an individual with a most significant disability in supported employment, that are:

(a)

Provided singly or in combination and are organized and made available in such a way as to assist an eligible individual to achieve competitive integrated employment;

(b)

Based on a determination of the needs of an eligible individual, as specified in an individualized plan for employment (IPE); and

(c)

Provided by the Program for a period of not more than 24 months, except that period may be extended, if necessary, in order to achieve the employment outcome identified in the individualized plan for employment (IPE).

(2)

Ongoing support services:

(a)

Are furnished by the Program from the time of job placement until transition to extended services, unless postemployment services are provided following transition, and thereafter by one or more extended service providers through the individual’s term of employment in a specific job placement; and

(b)

Include an assessment of employment stability and provision of specific services or the coordination of services at or away from the worksite that are needed to maintain stability based on:

(A)

At a minimum, twice-monthly monitoring at the worksite of each individual in supported employment; or

(B)

If under specific circumstances, especially at the request of the individual, the individualized plan for employment (IPE) provides for off-site monitoring, twice monthly meetings with the individual; and

(c)

Are provided for up to 24 months, except that period may be extended if necessary, in order to achieve the employment outcome identified in the individualized plan for employment (IPE); and

(3)

Specified in the individualized plan for employment (IPE); and

(4)

Include post-employment services when required to ensure that the employment outcome remains consistent with the individual’s unique strengths, resources, priorities, concerns, abilities, capabilities, interests, and informed choice.
