Hazard Management Process


(1) The RTA shall develop and document as a part of its system safety program plan a process to identify and evaluate safety risks and resolve hazards throughout all elements of the system, including the engineering and construction phase of any system, and including any hazards resulting from subsequent system extensions or modifications, operational changes, or other changes.
(2) The hazard management process must, at a minimum:
(a) Define the RTA’s approach to safety risk identification and evaluation, hazard management and the implementation of an integrated system-wide hazard resolution process;
(b) Specify the sources of, and the mechanisms to support, the on-going identification of hazards and safety risks;
(c) Define the process by which the identified hazard(s) and risks will be evaluated and prioritized for elimination or control;
(d) Detail the strategies designed to minimize the exposure of the public, personnel, and property to hazards and unsafe conditions;
(e) Identify the mechanism used to track through to resolution the identified hazard(s);
(f) Define minimum thresholds for the notification and reporting of hazard(s) to the SSOA; and
(g) Specify the process by which the RTA will provide on-going reporting of hazard resolution activities to the SSOA.
(h) At a minimum, the RTA must report to the SSOA the following hazards:
(A) Close calls involving:
(i) Employees or contractors working in the right-of-way;
(ii) Near-miss collisions between revenue and non-revenue vehicles; and
(iii) Industrial incidents with the potential for fatal or serious injury.
(B) Rule violations that may result in a catastrophic or serious mishap, including but not limited to:
(i) Switch run-throughs;
(ii) Operating through damaged or broken switches;
(iii) Doors opened on incorrect side at platform;
(iv) Doors opened off platform;
(v) Failure to comply with flagging or work zone restrictions;
(vi) Train order violations that may have a safety impact;
(vii) Unauthorized train movement near or through work zone;
(viii) Unauthorized and/or unprotected equipment or material that fouls or obstructs train movements on an adjacent track;
(ix) Train speed violations;
(x) Track right violations;
(xi) Signal violations; and
(xii) Radio violations that may have a safety impact.
(C) Malfunction or failure of safety-critical systems that could result in a catastrophic or serious mishap, including but not limited to the following:
(i) Tracks;
(ii) Broken rail;
(iii) Track buckle or thermal misalignment;
(iv) Placement of speed restriction;
(v) Train control system;
(vi) Wrong side signaling failure (false proceed);
(vii) Grade crossing warning system activation failure, partial activation or false activation;
(viii) Local or system-wide malfunction of the sign system or system component;
(ix) Traction power system;
(x) Substation;
(xi) Overhead contact system;
(xii) Failure of insulators and/or contactors;
(xiii) Failure of system components resulting in electrical arcing; burning, or smoke;
(xiv) Downed or loose live wires;
(xv) Light rail vehicle conditions;
(xvi) Broken wheels or tires;
(xvii) Broken axles;
(xviii) Loose or hanging components that may cause a train to derail;
(xix) Braking failures;
(xx) Doors off hinges;
(xxi) Doors open during train movement;
(xxii) Arcing electrical components; and
(xxiii) Occurring at passenger and non-revenue facilities.
(3) The RTA shall report any other hazards, as determined by the SSOA, resulting from inspections, investigations, audits or other reviews or observations that the SSOA determines to be a safety issue.
