Certificate prima facie evidence of right to practice

(1)

The Health Licensing Office shall issue a certificate to each applicant who qualifies under ORS 690.046 (Practitioner certification requirements) or 690.047 (Waiver of educational requirement). The certificate is prima facie evidence of the right of the holder to practice in a field of practice for which the holder has qualified and purports to be a practitioner.

(2)

A certificate must show the practitioner’s name, certificate number, expiration date and field of practice for which the practitioner is qualified to perform services.

(3)

A practitioner’s certificate shall indicate any specialty certification, obtained under ORS 690.047 (Waiver of educational requirement), for which the practitioner is qualified to provide advanced services in a field of practice. Advanced services shall be defined by rules adopted by the office.
