The exceptional circumstances requiring the use of a planned special exposure; and
The name of the management official who authorized the planned special exposure and a copy of the signed authorization; and
What actions were necessary; and
Why the actions were necessary; and
How doses were maintained ALARA; and
What individual and collective doses were expected to result, and the doses actually received in the planned special exposure.
The licensee must retain the records until the Authority terminates each pertinent license or registration requiring these records.
Upon termination of the license or registration, the licensee or registrant must permanently store records on Authority Form Y or equivalent, or must make provision with the Authority for transfer to the Agency.[ED. NOTE: Forms referenced are available from the agency.]
