Submission of Complaints


(1)

Any complaint filed with the Department of Consumer and Business Services by a pharmacy, or by an entity acting on behalf of a pharmacy, alleging a violation of ORS 735.530 (Definitions for ORS 735.530 to 735.552) to 735.552 (Pharmacy claims audits), shall be in form as posted on the Department’s Division of Financial Regulation website.
(2) A complaint shall include documentation of the alleged violation and of all efforts made to resolve the alleged violation prior to filing of the complaint.
