Process of Registration


(1)

The application for registration must be submitted to the Board by the first day of the month in which the license is due to expire.

(2)

The application for registration must be accompanied by the appropriate fee as listed in 847-005-0005 (Licensure Fees).

(3)

At its discretion, the Board may waive the fee for good and sufficient reason.

(4)

If the licensee is the supervising physician of a physician assistant or the primary supervising physician of a supervising physician organization for a physician assistant, the application for registration must include any updates to existing practice agreements or Board-approved practice descriptions for every physician assistant the licensee supervises.

(5)

If the licensee has been out of practice for more than 12 consecutive months or there are other concerns regarding the licensee’s medical competency or fitness to practice, the Board may renew licensee at Inactive status once the license renewal form has been completed satisfactorily.

(6)

The Board must provide to all licensees who have complied with this section a certificate of registration, which must be displayed in a prominent place in the licensee’s primary practice location through the end of the last business day of the registration period.
